Auto Ads Remove

Hi!
I want to remove the auto ads too.

My domain: doom.profusehost.net

And I have some questions:

I have a index.php that uses "include" function to insert all the others pages into it. Do I have to put the ads code in all this pages or just the index? Cause it will keep the ads code in all pages cycle.

And I must use 2 ads code? One in footer and other in the side/header?
